How do I retrieve data files from flight logs and interpret?

I just recently had a mishap while trying to take off with my hexa (APM 2, 2.5.4 version).  It flipped over, and I need to know if it was a software/hardware problem, or if I accidentally had it on the wrong flight mode, like loiter.  Can someone show me how to access the files from mission planner.  When I tried it, it said "error, fail to write files" or something of the sort.  What does that mean?  Please advise.  Thanks

do you have a dtaflash log or a tlog? recorded via radios

You mean I have to have radio such as xbee to get telemetry recordings?  If so, I do not, probably would not be a bad idea to get it as soon as possible.

I have tried to look at the tlogs in the log files from Mission Planner/ flight data/telemetry logs and always get the ERROR: Failed to write the log file. Can I access the tlogs from mission planner?

EDIT: I found a wiki on the tlogs and it appears that even though they are in logs folder on the computer they can only be read thru the wireless. Will work with that.
