Quadcopter 3.2.1 fly away and crash

Hi guys,

So on Sunday I had a bizarre accident with a quadcopter that I have been building for some time now. I built the quad for a friend and he had an issue where it attempted to fly away and crashed into a tree... So I rebuilt, replaced a motor and prepared for flight... after some weird flipping incidents I reflashed the APM 2.6(chinese knockoff) and recalibrated everything. I then sent a basic mission from Droid Planner on my phone... Quad kept flipping over so I tried sending the command from my PC, this worked and the quadcopter flew well the first time.

After the first flight I created another mission and sent to the quadcopter, I then attempted to send the mission to the vehicle, it reached waypoint 4 in the sending process and then I got an error, after this error I retried sending and it finally sent. I then armed the quad, flipped to Auto and the aircraft took off...

Now first two waypoints I realised that the drone was going a bit faster than normal and it was a bit jerky, the wind was a bit high but it wasnt much different from the first time I flew which went smoothly. I overlooked this and watched as the quad reached waypoint 5... then everything went wrong. The drone stood still for a moment but instead of RTL as it was programmed to do with the waypoint it took off flying in the opposite direction. I panicked and switched to stabilize afterwhich the drone literally fell out of the sky(throttle was low), I only managed to bump it up a bit before it landed into a coconut palm tree...

Damage was extensive - A damaged motor, a broken aluminum motor arm and badly bruised propellers.

I am trying to figure out why this happened before I rebuild. I do realise that many times the artificial horizon shows off(like showing the drone tilted when it is not) and sometimes it spirals(as if the drone is spinning when it is not) before being armed and I have to unplug battery and replug in order to reset everything. Included is my log. 

Would really appreciate any help I can get
