Trouble connecting to APM

i recently got a new computer now i cannot connect to the APM its asking me for A host name/ip i dont know what that is and then its asking for remote port can someone please help ?Continue

2 if you turn it to manual then back to Auto it also goes to the first waypoint
or in the Mission planner you can click on the restart mission button ."
