The CW motor was spinning faster because one of the Quick Detach Propeller adapter was broken and the prop is not "locked" down to the motor.
Change the adapter and it fly well.

Hi everyone,
I have problem on my quad and really require your advise..
Extreme Yaw/rudder on take off after repair from previous crash.
Previous crash was because of lost propeller on normal hover, it was hover quite stable before lost one of propeller.
Video on yaw problem when take off:
Log file with 3.1.5 + IMU added:
Flight Log with IMU
EDIT:
Motor 3 & 4 was spin faster than motor 1 & 2 as shown on below log:
New Log with Motors, IMU, etc.
Check list performed:
Suspect on one of ESC's or motors was broken during the crash, but how to determine which one is broken?
If any esc or motor were broken, why it still can take off from the ground?
Usually, quad will always tip-off if one motor/esc was failed.
Any ideas before I dismantle the entire quad?
Many thanks in advance.

Check your motors.. One of them is rotating in the wrong direction, Hence the yaw...
Thanks for the alert John, but I can confirm that all motors and props direction were checked several times according to guide below:

As long as you got there...
