After make px4, just do make px4-upload, no you shouldn't have to compile px4fmu directly form firmware directory to run make px4 in ArduPlane folder(you just have to do make configure_px4fmu form firmware directory). Yes, for the IO, you have to compile px4IO separately(do make distclean and make confifure_px4io and then make) and upload it to in your sdcard(px4io.bin). Then use the arming switch to program it once.
