There is a tutorial on rcgroups that shows how to build the cable and the pinouts for it. Also shows how to jumper the pins to power the whole board with just the 5v input, and has a trouble shooting section.
Is this on APM 2.5/2.6? I am using it on a Pixhawk, but there is two serial ports so no issue. On my APM 2.6 I am using just the radio (actually I am using an RDF 900 - but they work the same).
Apparently you can just plug in both with a Y cable. The current ones online though are for the old radio. Same principle, just a different plug I think.

Sorry, didn't realise. The pins are on the web site - see the Hardware section for PIXHAWK/APM2.x and pins are written there for each plug.

Do you mean pinout for telemetry cable port?
From the edge:
- GND
- NC
- Rx (to Tx on radio OR OSD)
- Tx (to Rx on radio AND OSD)
- +5V
