Crashed after switching to Alt Hold

I was flying the other day and was coming in to land in Stabilized mode, I switched to Alt Hold and it was as though the throttle was cut and the quad started to fall, I switched back to stabilized in hopes to recover and by then it crashed.  A review of my tlog (didn't know APM logging wasn't on) shows that the APM THOUGHT the quad was going up in altitude... both the GPS alt and Baro show an increase in altitude which I guess would explain the drop in Alt Hold... but any thoughts as to what may have caused this incorrect interpretation of altitude on both the GPS and Baro?

A little background on me and my quad.  It is a DJI F450 Flame wheel frame with TBS motors, APM 2.5 running firmware 3.1.5.  I just recently upgraded to the APM from a KK2 and have been tuning for the past few days of flying.  My Altitude Hold hasn't been holding very well so my main goal of this flight was to tune Alt Hold.  My flights were mostly alt hold and would start hovering and then give a good input in one direction and see how well it maintained height during flight as well as when I went back to center stick.  

My VCC hasn't been calibrate so it shows a little low (not sure what it actually is).  The APM is powered by a 3DR power module with the jumper removed.  I don't have a gimbal or anything else drawing 5v.  My FPV is all powered straight from my 3S 5000 mAh battery.
