While flying today, my quad flipped and fell out of the air. I'm thinking it was due to one of the motors shutting off. Do you think this is what it looks like. I've included a picture of the motor commands as well as my log.

I think you've nailed the reason.
That is certainly what it looks like to me, although I am not one of the experts, but a motor output going to max is pretty indicative.
