Flip on takeoff after bad crash

I recently had a very bad crash with my quadcopter (pilot error) which had previously been flying well.  Ever since the crash (and repair), I have been unable to take off.  I experience a flip to one side or another on takeoff.

I am running an APM 2.6 with Arducopter 3.2.1.

I would appreciate any tips on what to look for, how to analyze the log, etc.  Here is what I have checked:

* Motors are spinning in proper direction

* Props are mounted correctly (cw / ccw)

* Re-calibrated the Accelerator 

* Recalibrated ESC

* Even tried reflashing firmware and starting all over from the beginning

* All connections seem solid

* Radio signals are being received correctly (radio test)

As a test last night, I powered on quad, brought up throttle slowly until it was just about to take off -- everything seemed fine, pushed the throttle a bit, and the quad went in the air about 2 meters, dipped to the front and left (#3 arm went low -- fyi, this is the same arm that broke off previously).  Quad did half  flip and landed on it's back breaking two props on the way.
