Can anyone suggest why this was happening? Its random, sometimes after power on I got this, even after a few reboots, and then it goes away.

I armed the motors once when this was happening and it caused the quad to flip over even though the throttle was at minimum.

You'll have to tell us more. Have you calibrated with the CLI, did you let the ADC/leveling process finish before testing, what code are you using, etc.
