I have spent a very frustrating day trying to get Planner to work with my new APM2560 board.
The board seems to be OK. The proper sequence of lights, correct response to RC transmitter, FBW A response is correct to movement.
When I tried Simulation I got very strange results. Sudden disconnects, Planner crashes, etc.
I took some notes:
Loaded APM 2.2.1 HIL with Planner
Did 'First Time Setup' and set Modes to Manual, FBW A, AUTO
Configuration > Loaded .param file (Verified values)
Flight Planner > Load WP.h file (Verified flight plan, commands)
Started X-Plane with aircraft in position > paused
Simulation > Start (verified gains)
Planner > Flight Data "Heading to WP Home" Set Home Alt
Un pause X-P, > Manual take off > Switched to Auto ... CRASH!
X-P > reset aircraft Location. Reset AMP. Getting "Warning - No data for ... sec.
Orange TX LED blinking.
I then sent you information via DIY 'Send Message" I don't know if you got it or not.
Decided to try one more time with the same results > CRASH (Simulated)
Decided to try to configure for normal flight.
Quit X-Plane Telemaster power cycled
Restart Planner
Firmware > Connect Download APM 2.2.1
Hit 'First time' button EEPROM erased > radio cal done > Logs cleared >
Terminal > setup > modes (Man, FBW A, Auto) Return > Reset APM
Verify Modes. Exit Terminal > Configuration Load Cadencia 07-01-11 FLIGHT.param
Write Pids >

Could you do an manual "erase" via the cli. and "reset"
also how are you connecting usb? or xbee?
