I booted APM - now with stock Plane 2.33 firmware - but this problem existed for a very long time.
Altitude = 0 (real altitude is less than 20meters.)
waited for GPS fix.for several minutes.
Altitude is now 82 meters. (the moment that GPS got fix.)
rebooted APM (reset)
Altitude is now 44 meters.
rebooted APM (reset)
rebooted APM (reset)
still stuck at 44m

how are you reading out the altitude? Depending on what software you are using, you are probably reading out the changes in the GPS altitude. GPS altitudes are notoriously bad, and often vary a lot over a short time.
For flying waypoints, ArduPlane uses barometric altitude by default, and that should only be inaccurate if you have a strong airflow over the barometer.
