My Arduplane (2.73) + HK Bixler performs well on autopilot missions with regard to hitting waypoints in lat/long, but sometimes the altitude can be way out.
I made some anti vibration modifications, and shading of the PCB from sunlight, which improved things. But is still can be a bit out - especially on the first few waypoints when climbing at the start of a mission.
Is the WP radius setting in flight planner in 3 dimensions (i.e. set to 40m is that in lat, long and altitude?)

What kind of GPS do you use?
Sometimes the GPS altitude can be off, and that is the altitude you see in mission planner.
For navigation the APM relies solely on barometric altitude.
if it continues to climb during your mission you might need to increase your nav alt P gain . see if that helps.
Also make sure the APM is shielded from prop wash and wind. Use the 3DR case if you aren't already. the foam in the case is a real good protection for external influences.
