It just fell out of the sky

I have been having a lot of problems when ever I use loiter. The results are always unpredictable.

Sometimes it holds and some times depending on the wind I get quit a bit of oscillations I have spent a long time tuning but can get it dialed in. It fly's perfect in stabilize, acro and alt hold.

Was doing some the testing and it just went horizontal and hit the ground. Attached are the logs any help would be great. Starting to pull my hair out.

    Can you tell us more about your setup? 

    1. Have you mounted the APM on vibration-damping foam?
    2. Are you using APM 2.6 with the remote GPS/mag or if not, have you mounted APM very high above source of magnetic interference?
    3. Can you tell us what frame/motors/battery/props you're using?
