I had this same question yesterday. After much research, I found a rely by Jason to someone else's question about changes in v43. I may not get it fully, but here's my take on it:
Simple mode modifies what direction the copter flies. Instead of forward being defined by the direction of the APM (the ABC LEDs are on the "Forward" side of the APM board), forward is the direction you want to go by pointing the right stick in that direction. This is great when you can't tell what direction the copter is pointing (which legs are pointing forward). It no longer matters! And the"simple" modifier can apply to any mode. It took me a while to understand that statement too.
I think this is a case where the code is ahead of the documentation.
did you look at the Configuration->Setup->Modes screen in Mission Planner? It became a modifier for the modes, rather than the mode itself. Stabilize w/ Simple, Loiter w/ Simple, etc.
