Help needed analyzing the cause of a crash!

Yesterday I was testing Simple Mode. I added a mix for CH8 to use a 2-pos toggle switch on my radio to turn it on and off. At first we took off and flew in STABILIZE, ALT_HOLD and LOITER modes. While in ALT_HOLD I activated Simple Mode. I confirmed that Simple Mode worked as expected. In testing this mode I flew the Y6 away from me far enough that it triggered RTL (ERR Subsys=9, ECode=2 - Circular fence breached). It this point all I expected was for the Y6 to climb to the set altitude and return to home position. Although it did climb somewhat, it also rolled left and went into a dive and crashed. There is a lot of data in the log but I'm not sure how to determine the exact cause. I can see the ThrOut pegged at 1000 while the ThrIn is significantly lower. I'm not sure what this means but I it doesn't appear to be normal.

 

Something else I noticed is that when I changed to LOITER mode, the copter started circling. You can see this in the picture LOITER.PNG (Yellow and Blue). Why would this happen?

 

I've included the log as well as some pictures hoping some can shed some light as to why the crashed occurred.
