"I have done the power filter as well as used homemade shielded cable (Wrap alu foil around the servo cable and used shrink tube). I don't see any difference in the ALT_Hold. Sad to say but It even got worst. :(

If you post the Flash Logs I can help. Most likely your sonar is blocked.
You should make sure you have flown in Stabilize for about 10 seconds to narrow in on your throttle hold center point for any alt hold.
