"Listen to Helldesk and Jasonshort.  And double/triple/quadruple-check your prop direction and motor direction and ESC to APM cabling. If your quad wants to flip over at or before liftoff, it is frequently a swapped set of motors at the ESC,…"

"I'm guessing you are also using the latest version of the Mission Planner too then, but just in case the latest version 1.0.76 is available here. What I would do next is going over the motors, propellers and which motor is plugged in to which…"

"EDIT FOR CLARITY: You might have one of your ESCs OUTPUTS connected the wrong way, so the propellers spin the wrong way. There are three cables coming out of the ESC on the output side. The two outer cables can be reversed to change the direction…"

"Adam, you can't reverse the polarity of a brushless ESC without frying them (or, at best, not making the motor spin at all). Brushless motors are reversed by swapping any two of the three wires between the ESC and the brushless…"

"Also, you should use the mission planner to confirm that your radio's stick inputs are correctly interpreted by the board. Sometimes you might have a channel reversed, which will cause it to look like it is rolling to the side hard because you…"
